4th Step Forms
The links on this page will download forms for use with the 4th step. The forms track the language of the Big Book of Alcoholics Anonymous and break the fourth step down the way it is described in the Big Book.
INSTRUCTIONS: The forms are provided in Microsoft Word 2000 format. To download, click on a link (even on a fast connection, it may take a moment). If you are prompted for a user ID and password, click "cancel" (Note: the dialog box for user ID and password may appear behind your Web browser - click on the "Enter Network Password" button on the task bar at the bottom of your window to bring box into view, then click "cancel"). Depending on what Web browser you are using, clicking on the link will either cause the document to open in Word as a separate window or with Word integrated into your browser. To save the document on your hard drive, click "file" and "save" and then choose a directory or folder location for the document.
